  • May create operational, financial or workplace risks

  • Is difficult to reverse

  • Requires support from others to succeed

  • Involves a new process, policy or technology

  • Needs to be communicated and implemented carefully

The 10-page fillable workbook guides leaders through five practical stages:

  4. Evaluate options and risks

  5. Make, implement and review the decision
     

It includes guided questions, interactive checkboxes, generous answer areas, an implementation plan and a final decision check.

Complete it electronically, save your answers or print it and complete it by hand.

How to Download and Use the Checklist

  1. Select the download button.

  2. The PDF will open in a new browser tab.

  3. Select the download icon in the PDF viewer.

  4. Save the blank checklist to your device.

  5. Open the saved copy in Adobe Acrobat Reader or another application that supports fillable PDFs.

  6. Complete the applicable fields and checkboxes.

  7. Save the completed checklist under a new filename.

Privacy and Confidentiality

Information entered into this checklist is not submitted to R. G. Virtual Group or collected through this website. It remains in your copy unless you choose to share the completed document.
 

The checklist does not contain a submission feature, and you do not need to return the completed checklist to R. G. Virtual Group.
